Chiefton report outlines many recommendations
The UK Marine Accident Investigation Branch (MAIB) has published its very detailed report into the investigation of the collision, capsize and foundering of the tug Chiefton with the tragic loss of one crew member, on the River Thames on 12 August 2011.
